Online Registration for the 2009-2010 season is NOW OPEN!

We are using a different online registration system this season - your old QuickEnrollment

user ID & password will not work in the Hockey Canada online registration system.

Important Message Regarding our Midget House League Program

Starting in the 2009-2010 season, our Midget House League Division will become a Midget - Juvenile Division.

This division will be open to all Midget age players as well as all 1st & 2nd year Juvenile age players. This will be a NON-checking division.
Select programming will continue to be available at the Midget age and the Midget Select programming will continue to include body checking

GENERAL REGISTRATION INFORMATION: For players born between Jan. 1, 1989 and Dec. 31, 2005		Click here for information about our Pre-Novice Program
Basic registration fees:	Pre-Novice to Midget /Juvenile		Juvenile Travel players
Basic registration fees:		Regular Registration	Juvenile Travel players
1st Child		$460	$300
2nd Child		$435	$300
3rd & subsequent children		$410	$300

$75 Out of Town Fee for players not residing in the Town of Fort Erie

Travel Fees - $275 per player (once selected for the team)

Select Fees - $250 per player (once selected for the team) - Please note that as requested by 2008 select coaches, this fee does not include tryouts, so there will be an additional fee for tryouts (TBA)

Players must reside in the Town of Fort Erie to play travel (OMHA residency rule).

Travel Tryouts will begin in late August or early September, check this page for dates & times

Players who do not reside in the Town of Fort Erie may play Select (Tryouts TBA) if they are entering

their third consecutive year in GFEMHA.

Payment options - VISA / MC, cheque, money order, cash or Debit

NOTE: PLEASE DO NOT SUBMIT PAYMENT UNTIL PLAYER(S) HAVE BEEN REGISTERED

Payments by cheque or money order may be mailed to:

� Payment in full must be received no later than September 30th

� We will accept post dated cheque(s) dated the 15^th and 30^th of each month up to September 15th

� After September 15th, we will accept online credit card, money order, certified cheque, cash or debit payments only.

� WE WILL NOT ACCEPT UNCERTIFIED CHEQUES DATED AFTER SEPTEMBER 15, 2008

Payment dates: Cheque, Money Order, Cash or Debit at the GFEMHA Office.

Payment dates and times are as follows:

�	Saturday, July 18th	11am to 2pm
�	Saturday, August 15th	11am to 2pm
�	SUNDAY, September 13th	11am to 2pm
Certified Cheque, Money Order, Cash or Debit ONLY after September 15th
�	SUNDAY, September 27th	11am to 2pm
�	During first two travel tryouts for each division

If payment in full is not received by September 30th a late charge of $50.00 will apply & player(s) will not be permitted to play until payment in full is received by online credit card, money order, certified cheque or cash.

Please include a copy of your invoice with your payment to avoid any confusion.

Mail Slot: Drop cheque(s) or money order (NO CASH) through the mail slot in the GFEMHA Office Door, located at the far end of the Kinsmen Rink at the Leisureplex. (No uncertified cheques dated after September 15th)

IMPORTANT NOTE: In order for your payment to be accepted, the player must ALREADY be registered online. We will NOT be doing registrations on these dates.

If you are going to pay by any method other than online credit card, you will have to select either 'cheque' or 'money order' - even if you plan to pay by debit or cash.

All returned payments (e.g. NSF, Stop Payment, Account Closure, etc.) will be subject to a $30 Administrative Fee and must be replaced within

10 days of notification by cash, certified cheque or money order. The player will not be permitted to play until replacement funds are received.

REFUND POLICY

� Refunds will be issued only upon written request.

� Please submit your request to GFEMHA to the Treasurer by email (treasurer@forteriehockey.com), fax (905-871-3457), or through the mail slot at the GFEMHA Office in the Kinsmen Rink at the Leisureplex.

� Provide the players name; name to make cheque payable and address to send refund cheque.

� Please allow approximately 4 - 6 weeks for processing from time of notification.

� Amounts paid by cheque will be delayed until cheque clears bank.

� There will be a $30 fee charged for NSF cheques.

� Administration fee of $25 will apply

� The balance of Registration fees will be refunded as follows:

On or before Oct 15^th � 75%; Nov 15^th - 60%; Dec 15^th - 35%; Jan 10^th - 20%; after Jan 10^th - 0%
